Confidential Reports and Recommendations
Below you will find the links to the appropriate Confidential Reports by age group or grade for your child's school to access.
4-5s through 6-7s (Pre-K, K, and 1st Grade)
Confidential School Reports: if your child is in a school setting (nursery, preschool, daycare, co-op, etc.), please submit the appropriate Confidential Report to your child’s current teacher(s) or preschool director. Teachers/directors should return the filled out forms directly to MCS no later than January 12, 2023.

7-8s through 7th Grade
1) Confidential Teacher Evaluation:
Please submit the appropriate form to your child’s current teacher(s). Teachers should return the filled out forms directly to MCS no later than January 12, 2023.
- Confidential Teacher Evaluation for Children Applying to 2nd through 4th Grade
- Confidential English Evaluation for 5th Grade and Up
- Confidential Mathematics Evaluation for 5th grade and Up
- Permission to Release School Records
2) School Reports:
- Families applying to 6-7s (First Grade) must arrange to have MCS receive Kindergarten school reports.
- Families applying to 7-8s (Second Grade) and up must arrange to have MCS receive the current year and previous year school reports/transcripts from your child's current school.
